Student Background Program
- Student Background Check
Overview
The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) now requires
all students, volunteers and employees in participating facilities to undergo a
background prior to being accepted into any program in a medical facility (January 2004).
This is not a state or federal law, but rather a rule implemented by JCAHO to ensure
patient safety.
Adam Safeguard has responded by instituting a student program:
• The student navigates to our website and enters their information (many schools provide
the data on their own website)
• Payment is taken directly from the student in the form of a credit card or money order
• Adam verifies and validates the Social Security Number, as well as running a required
Statewide Criminal in New Jersey, and other states where applicable. If a discrepancy
is found with either the Social Security Number or the Criminal History, Adam will
provide necessary information to the student on how to dispute/correct the record.
• Upon completion of the report, results are emailed automatically to the educational
institution and the student simultaneously.
School Access
• Schools will be provided with paperwork detailing the program complete with turnaround
times and pricing to hand out to the students.
• The results are maintained in a database indefinitely, with the educational facilities
having complete, 24/7 access.
Program/Payment Information
Base Package (New Jersey Statewide Criminal History Search and SSN Trace):
Social Security Number Trace
Provides validation that your Social Security Number (SSN) corresponds to your name.
The report will also indicate any other names that are associated with that SSN and
provide information as to the state and year of issuance along with present and past
residences. This trace will act as a guide to determine the location(s) to conduct
the Criminal History search. If a discrepancy is found in this record, we will
provide you with the necessary guidance to identify and correct the problem.
Statewide Criminal
The New Jersey Statewide Criminal History Search is a Felony/Misdemeanor record search
by name, DOB, and Social Security Number of all 21 counties in New Jersey. Results
will be returned within ten (10) business days. Hits (criminal records) will be returned
within two business weeks after the initial 10 business days.
Pricing/Payment
The charges for additional states are as listed and are in addition to the $60 base
package price:
• NY - $72.00
• PA - $30
• All other states by county - $20 per County plus additional State Fees if applicable
• Adam Safeguard will accept Money Orders, or Visa, Mastercard and American Express
• The charges on your credit card will reflect the name Adam Safeguard.
• For those not utilizing a credit card you should complete the form, print it and
send it with a bank check or money order to Adam Safeguard at PO Box 1210, Toms River,
NJ 08754.
• We ask that you do not contact Adam Safeguard with inquiries regarding completion
of reports before the 10 business day- turnaround time has passed.
Although we are approved by your school, you are our client. You are entitled to
our best service, our support, unlimited copies of your report, and indefinite profile
storage.
Student Instructions
- Students will go to www.adamsafeguard.com and select the link titled "Student Backgrounds",
and select their school from the list.
- Once at the order page, they are required to fill in their personal identifiers and
select a method of payment. (Money Order or Credit Card)
- Once payment information is entered, the student will be directed to an order page
that outlines what venues will be searched as well as the total price. At this point,
the student is asked to purchase the report ONLY if they agree to the areas being
searched and the price.
Frequently Asked Questions
• Will I be charged if I have lived outside of NJ? If so, how much?
- Yes. The charge is $15 for each state outside of NJ (excluding NY & PA). The charge for
a NY Search will be $71.50, and the charge for PA will be $30.
• Will my prior arrest, criminal history, conviction, etc. show up?
- We do not know what arrests, convictions, etc. will show up until the searches have been
run.
• How long does the search take?
- The search will take about ten business days to complete. Unfortunately we cannot expedite
this process, nor can we contact the State Police for updates on results.
• What crimes/convictions/arrests will keep me out of the program?
- The medical facilities will determine which students they will accept and deny
• Who will have access to/see my report? My credit card number? My SS Number?
- Adam Safeguard will have access to all three indicated above. The school and the medical
facility will have access to the reports, which will contain your SS Number.
• When/How will the school receive my report?
- The school will be directly linked to our system and they will be able to log in and
view your report as soon as it is completed.
• What do I do if an arrest/conviction shows up on my report and it is not me?
- Adam will provide you with information on how to dispute/deny/admit/correct any
discrepancies in your criminal history. These are one page forms that can be found

- You can find information on how to correct this discrepancy: 
• What if I had a PO Box in another state but never lived there?
- We will still need to run a criminal search in that state, because the general idea
is that you still had the opportunity to commit a crime in that area, resident or not.
What to Do When a Criminal Record is Discovered in Your Report
1. If it is your record -
A. Make sure all the details and findings are correct, if not contact the authority
to correct any defects (i.e. 60 day jail time was actually suspended).
B. Criminal records are not automatically a reason for denied eligibility.
Therefore it is important to prepare a statement as to the circumstances involved
in your issue. This may include indicating your age at the time, how long ago the
event occurred, proof of rehabilitation and details or reasons things went wrong.
This statement is only to be presented to the accepting institution when necessary.
2. If you dispute the record -
A. Verify all identifiers, name, middle initial, DOB, SSN, and any address information
as well as the location of the record (were you ever there?).
B. Contact the reporting authority, agency, or court for accuracy or any discrepancies.
C. Check dates of arrest versus your whereabouts.
- See if the arresting agency has a photo (mug shot) of the person.
- Contact the agency or officer for details which may prove its not you, even offer a
face to face meeting for identification.
- If you are able to demonstrate someone else was the party, have the court correct
the record and provide you with a document with the explanation.
- If you wish you may contact and attorney to assist your efforts.
D. In the unlikely event that a criminal record has come back from the State
Police matching all of your personal identifiers (i.e. name, DOB, SSN), you will need
to contact the NJSP and you will then be required to submit your fingerprints.
Providing the record is clear, they will then provide you with an official document
stating that your record is clear and someone else has assumed your identity if that
is the case.
3. In any event you will need to confirm your eligibility from any medical facility that you will be involved with. Each institution will make a decision on a case by case basis.
Your Rights Regarding Your Social Security Number
What do you do when someone else’s name appears under your Social Security Number?
• Verify the SSN in question was entered correctly
• Match the SSN to the Social Security Card
If the above steps were taken and there still is a discrepancy, than you should:
• Contact all three major credit bureaus listed below to notice them of the error.
• You are entitled to run your credit report once a year for free from each of the 3 bureaus.
• You may also visit www.annualcreditreport.com
to obtain your free 3-bureau credit report
• Once you have done that you may also contact the Social Security Administration
- www.ssa.gov or 800-772-1213
- You can also check your earnings record by calling the 800 number and asking
for a Form SSA-7004, Request for Earnings and Benefit Estimate Statement.
The Statement will show the earnings reported to your Social Security number
each year since 1951. If the information on your earnings record is incorrect,
they will help correct it. You can make an online request at this site:

Taking these steps can prevent Credit Card Fraud, Identity Theft, and simply avoid
confusion in the future.
College Instructions
•Colleges/Educational Facilities will be provided with a username and password and directed to
Adam Safeguard’s ordering system.
•Administrators and authorized personnel will have 24 hour access to student records over our secure site.
•Profiles are emailed to authorized recipients upon completion, and results are stored indefinitely.
•A healthcare student usually participates in programs in more than one facility.
Since each facility must require proof of a current background, Adam will act as a clearing house
or central database on the students’ behalf, storing and accessing the profiles as needed.
•Adam will accept payment from the student or the facility.
